REQUESTFORSERVICESPage1of3NameofClientAddressTelephoneno.Faxno.EmailAddressService(s)requiredFeesUS$225.00/hour(survey/officetime)andUS$100.00/hour(traveltime),plusexpensesatcost,includingdailysubsistenceofUS$25.00incurred.At(location/site)Thisrequestforservicesissubjecttothetermsandconditionsonthisdocument.Pleasesignandreturnthisrequestforservicestoyourlocaloffice.TERMSANDCONDITIONSThefollowingtermsandconditionsapplytoallservicesprovidedbyanyentitythatispartofthe“LRGroup”ashereinafterdefined:1.Inthesetermsandconditions:(i)“Services”meansanyandallservicesprovidedtotheClientbyLloyd’sRegisterofShipping(“LR”),oranyentitythatispartoftheLRGroup,ashereinafterdefined,includinganyclassificationoftheClient’svessel,equipmentormachinery;(ii)the“Contract”meansanyagreementforsupplyoftheServices,suchasarequestforservicesoranyotherdocumentoragreementrelatingtotheprovidingofServices;and(iii)the“LRGroup”meansLR,itsaffiliatesandsubsidiaries,andtheofficers,directors,employees,representativesandagentsofanyofthem,individuallyorcollectively.2.TheClientwillpayallinvoicesfortheServiceswithin30daysoftheinvoicedate.TheLRGroupentitythathasprovidedtheServicesreservestherighttochargeinterestatanannualrateof2%abovethegreateroftheLondonInterbankOfferedBaseRate(LIBOR)ortheequivalentinthecountrywheretheClientmaintainsitsprincipalofficeonanyamountremainingunpaidbeyond30days,andmaywithholdanyorallServicesuntilthearrears,includinginterest,arepaid.3.TheLRGroupentitythatprovidestheServicesreservestherighttochargeforanyworkthatisadditionaltothatoriginallyquoted.4.TheLRGroupwillkeepconfidentialandnotuseordisclosetoanythirdpartyanytechnicalinformationoroperatingdataderivedfromtheClientinconnectionwiththeServicesexceptasmayberequiredbylaworasmayberequestedbytheClient.ThisobligationwillsurviveterminationoftheContract.ThisobligationwillnotapplytoanytechnicalinformationoroperatingdatathatwasintheLRGroup’spossessionbeforeitsdisclosuretotheLRGroupentityinconnectionwiththeServices,thatisorbecomespartofthepublicdomainthroughnofaultoftheLRGrouporthatotherwiselawfullybecomesavailabletotheLRGroupfromanindependentsourcenotunderaconfidentialityobligation.5.TheContractbetweenanLRGroupentityandtheClientcontinuesinforceuntilterminatedbytheLRentityortheClient,aftergivingtheotherparty30days’writtennotice.6.IfaContractisterminatedbyanLRGroupentityortheClientbeforetheServicesundertheContractarecompleted,theLRGroupentity’sfeeswillbecalculatedonaproratabasisuptothedateoftermination.AnyreasonablecostsdirectlyattributedtoearlyterminationandanyamountsthenduetotheLRGroupentitywillimmediatelybecomepayable.7.LR’sservicesdonotassesscompliancewithanystandardotherthantheapplicablerulesandcodesofLloyd’sRegisterofShipping,internationalconventions,oranyotherstandardsthatareexpresslyagreedinwritingbyLRandtheClient.Withoutlimitingthegeneralityoftheforegoing,theissuanceofaclasscertificatebyLRdoesnotinitselfconstituteaguaranteethatthevesselisseaworthyanddoesnotrelievetheowneroroperatorofthevesselofitsnon-delegabledutytomaintainthevesselinseaworthycondition.Form2508(2002.09)Page1of3REQUESTFORSERVICESPage2of38.IftheClientrequiresclassificationservicesrelatingtovessels,machinery,orequipmentclassedbyLRinajurisdictioninwhichLRitselfdoesnotdobusiness(includingwithoutlimitationBrazil,Canada,Greece,andtheUnitedStatesofAmerica),theClienthe...
