老年糖尿病患者合并肺部感染的病原菌分布及耐药性研究【摘要】目的:分析老年糖尿病合并肺部感染住院患者的病原菌分布及其耐药性,以期对糖尿病合并肺部感染患者在临床治疗及用药方面提供依据。方法:选取2012年9月-2015年9月入本院内分泌科治疗的糖尿病合并肺部感染老年患者480例。搜集其痰标本进行培养,分离鉴定病原菌分布情况并行药敏性试验,统计所分离菌株的分布情况及耐药性特点采用法国梅里埃公司Vitek2Compact型病菌鉴定及药敏性分析仪(DADE公司)进行鉴定分析。结果:老年糖尿病合并肺部感染患者标本中共分离病原菌318株。其中革兰阴性菌216株,占65.68%,肺炎克雷伯菌(26.10%)、大肠埃希菌(16.35%)、鲍氏不动杆菌(11.63%)是其主要组成;革兰阳性菌80株,占25.16%,其主要病菌组成依次为粪肠球菌(13.52%)、金黄色葡萄球菌(8.49%)、屎肠球菌(3.14%);真菌22株,占6.91%,由白假丝酵母菌(5.03%)、热带假丝酵母菌(1.87%)组成。革兰阴性菌中肺炎克雷伯菌、大肠埃希菌、鲍氏不动杆菌、铜绿假单胞菌、洋葱伯克霍尔德菌、阴沟肠杆菌对头孢噻肟头孢他啶、阿米卡星、妥布霉素及阿莫西林等传统类抗菌药物耐药率较高(>40%),对亚胺培南、美罗培南等抗菌药物的耐药性较低(<40%),鲍氏不动杆菌对美罗培南、洋葱伯克霍尔德菌对雅安培南的耐药率均为0;革兰阳性菌中粪肠球菌、金黄色葡萄球菌及屎肠球菌对红霉素、利福平、青霉素及左氧氟沙星等抗菌药物耐药率较高(>40%),金黄色葡萄球菌及屎肠球菌对替考拉宁的耐药率为0。结论:老年糖尿病合并肺部感染患者病原菌分布以革兰阴性杆菌为主,其耐药性较强;临床治疗中应以病原菌分布及药敏结果为依据合理应用抗菌药物,达到减少新耐药菌株的形成,进而降低医院感染的发生。【关键词】老年糖尿病;肺部感染感染;病原菌;耐药性Elderlypatientswithlunginfectionanddiabetesofpathogenicbacteriadistributionanddrugresistance【Abstract】Objective:Analysisofelderlydiabeticlunginfectionofhospitalizedpatientswithclinicalfeaturesandthecharacteristicsofetiology,inthehopeofpatientswithdiabetescomplicatedbypulmonaryinfectiontoprovidebasisforclinicalapplication.Methods:Select9September2012-2015,worksinourhospitalendocrinologytreatmentofdiabetes,480casesofelderlypatientswithpulmonaryinfection.Collectingthesputumspecimensforculture,separationandidentificationofpathogendistributionparallelsusceptibilitytest,statisticaldistributionanddrugresistancecharacteristicsoftheisolates,theFrenchmerryEmmanuelcompanyVitek2Compacttypebacteriaidentificationanddrugsensitivityanalyzer(DADEcompany),sputum2timesinarow,itisconcludedthatthesamepathogencanbeusedastheetiologicdiagnosisbasis.Results:SpecimensofelderlydiabetespatientswithpulmonaryinfectionofthecommunistpartyofChinaseparationofpathogenicbacteriaof318strains.,ofwhich216strainsofgram-negativebacteria(65.68%),klebsiellapneumoniae(26.10%),e.coli(16.35%),bowman'sacinetobacter(11.63%)isthemainpart;80strainsgram-positivebacteria,accountingfor25.16%,ofthemainpathogensofdungenterococcus(13.52%),staphylococcusaureus(8.49%),excremententerococcus(3.14%).Fungi22strains,accountingfor6.91%,bythewhitecandida(5.03%),tropicalcandida(1.87%).Klebsiellapneumoniaeingram-negativebacteria,e.coliands.boydiiacinetobacter,pseudomonasaeruginosa,onionburkholderiabacteria,sewere.colitocefotaxime,cephalosporinsheorganism,amikacin,tobramycinandamoxicillintraditionalclassofantimicrobialagentssuchasresistancetohigh(>40%),oftheiminesouthernculture,e.faecalisandimidepeisouthofantimicrobialdrugresistanceislow(<40%),thebowman'sacinetobactere.faecalistobeauty,onionburkholderiabacteriatoyaculturesouthoftheresistantratetozero;Dungenterococcusingram-positiveb...
