SupplierSocialResponsibilityandBusinessEthicsAgreement供應商社會責任與商業道德同意書SupplierSocialResponsibilityandBusinessEthicsAgreement總則General佳世達供應商/“”分包商及其分供應商（以下簡稱供應商）承諾所有企業活動均在國際和當地適用法律法規的框架內進行，公平競爭，誠信交易，嚴格遵守商業道德，積極履行社會責任，並遵守電子行業公民聯盟(ElectronicIndustryCitizenshipCoalition,EICC)行為準則。Qisdasupplier/subcontractorsandsub-supplier(hereinaftercalled“supplier”)promisesthatallthecompany’sactivitiesareconductedinaccordancewithapplicablenationalandlocallaws,andshallcompetefairly,tradetrustily,complywithbusinessethicsstrictly,executesocialaccountabilitypositively,andcomplywithElectronicIndustryCitizenshipCoalition(EICC)CodeofConduct.環境Environment供應商應當出具ISO14001證書，或相關計畫以明示其正為獲得該證書而努力，或者擁有可操作性的環境管理體系。如以上均不具備，至少應該提供確實證據，以證明其環境狀況的持續性改善。SuppliershallhaveISO14001certificationoraplantobecomecertified.Alternativelysuppliermustprovideapracticableenvironmentalmanagementsystemoratleastdemonstrateexplicitandsuitableevidenceswhichshowcontinualenvironmentalimprovement.供應商應當與本公司簽署相關的禁用物質聲明。SuppliershallsignrelevantbannedsubstancedeclarationwithQisda.健康與安全HealthandSafety供應商應當為員工提供安全與健康的工作場所和環境，並承諾遵守OHSAS18001之要求。對於危險工作區域、有害材料或危險產品應當加以明顯標示。Suppliershallprovidehealthyandsafeworksitesandcircumstancesforworkers,complywithrequirementsofOHSAS18001,andidentifyandnoticehazardousworkareas,harmfulmaterialsanddangerousgoodswithdistinctmarks.供應商應當定期對健康安全風險進行評估，對於預期風險有相應地管理計畫或安全管控措施，以及緊急救助程序。Suppliershallassesshealthandsafetyrisksregularly,andshallhavecorrespondingmanagementplan,safetycontrolmeasuresandemergentresponseproceduresforFormNo.:QTY0-0D-004-20(150323)SupplierSocialResponsibilityandBusinessEthicsAgreementanticipatedrisks.供應商應當對所有員工一視同仁地宣導和教育，以確保員工知悉公司的健康安全計畫、風險和救助程序。SuppliershalltrainandcommunicatewithallworkersimpartiallytoensuretheirunderstandingofHealthandSafetyplans,risksandemergencyprocedures.供應商應當定期對健康安全計畫、程序、相關設施進行評估和改進。SuppliershallassessandimproveHealthandSafetyplans,proceduresandrelevantfacilitiesregularly.勞工Labor供應商應當承諾保障作業同仁的人權安全，讓員工受敬重並維持尊嚴，並承諾遵守SA8000及EICC行為準則之要求。Suppliershallpromisestoprotecttherightsofworkers,totreatthemwithdignityandrespect,andtocomplywiththerequirementsofSA8000andEICCCodeofConduct.自由選擇職業FreelyChosenEmployment供應商承諾不使用任何形式的強迫勞工、奴役及販賣人口。供應商不得扣留人員的部分薪資、福利、財產或文件檔案。所有工作均應當出於自願，員工擁有自由離職或終止僱傭關係之權利。Suppliercommitstonoforcedlaborofanyform,noslavery,andnohumantrafficking.Suppliershouldnotwithholdanypartofanypersonnel’ssalary,benefits,propertyordocumentsinordertoforcesuchpersonneltocontinueworking.Allworkmustbevoluntaryandworkersshallbefreetoleaveworkorterminatetheiremployment.禁止對員工進出入工作場所作出不合理限制，亦不得約束勞工在工作場所內走動的自由。Thereshouldbenounreasonablerestrictionsonworkers’freedomofmovementinthefacilityinadditiontounreasonablerestrictionsonenteringorexitingcompany-providedfacili...
