Facility and Merchandise Authorization Form (Attachment A) 34FACILITY AND MERCHANDISE AUTHORIZATION FORM SAMPLE Attached is the Facility and Merchandise Authorization Form (“FAMA Form”) that must be completed for each Facility that will produce Disney Merchandise. Please make as many copies of the attached FAMA Form as necessary so that you can complete a FAMA Form for each Facility Once the FAMA Form is completed for each Facility, please send to: Insert Name of Appropriate Person Title and department Disney Business Unit or Region Address line 1 Address line 2 Address line 3 Phone: Fax: E-mail: Disney will verify the information and determine whether each Facility will be allowed to produce Disney Merchandise For those Facilities that are allowed to produce Disney Merchandise, and have not yet executed a Manufacturer’s Agreement on behalf of Disney, you will be sent a Manufacturer’s Agreement that must be signed by each Facility Once it is determined that a Manufacturer’s Agreement is in place for each Facility, the FAMA Form will be signed by Disney and returned to you You may begin production of Disney Merchandise as authorized under your applicable agreement with Disney, once you receive the signed FAMA Form from Disney You may also present the Form to Customs officials to facilitate the importation of goods if the Facility is outside the territory where the Merchandise is to be sold. Emergency Customs letters will no longer be issued.
