手术治疗合并下胫腓分离的踝关节骨折【摘要】 目的 总结合并下胫腓联合分离的踝关节骨折手术治疗的效果。方法 治疗合并下胫腓联合分离的踝关节骨折 52 例，采纳钢板固定腓骨，螺丝钉固定内踝，下胫腓仍不稳定者应用单个皮质骨螺丝钉固定。结果 术后随访 8～36 个月，骨折愈合良好，依据 Olerud 评分方法，优 36 例，良 13 例，可 3 例。结论 手术复位是治疗合并下胫腓联合分离的踝关节骨折的必要选择，正确的复位顺序、合适的手术入路及固定方法是取得良好复位的关键。【关键词】 下胫腓联合分离；踝关节骨折；手术治疗 Surgical Treatment on the Disruption of Tibia fibular Joint Combined with Malleolar Fracture Abstract：Objective To summarize the effectiveness of the disruption of tibia fibular joint combined with malleolar Forty two cases were treated by The patients were followed up for 8 to 36 patients acquired bone follow up outcomes were determined using Olerud′s criteria，which revealed that 36 patients were excellent，13 good，3 The good effect of operated treatment is available on the disruption of tibia fibular joint malleolar fracture. Key words：disruption of tibia fibular joint；malleolar fracture；surgical treatment 对于部分 Weber B 型及全部 Weber C 型踝关节骨折合并有下胫腓联合分离的患者，手术治疗是必要选择。自 1999 年 8 月至2025 年 1 月，我科共收治 52 例合并下胫腓联合分离的闭合性踝关节骨折，现报告如下。 1 临床资料 一般资料 本组 52 例，男 36 例，女16 例；年龄 18～71 岁，平均 岁。受伤至就诊时间 30 min～10 d。按 Danis Weber分型方法，B 型 21 例，C 型 31 例。 治疗方法 连续硬膜外麻醉，气囊止血带下操作。依次暴露内踝、后踝、外踝，然后按后踝、外踝、内踝顺序整复。自内踝尖上 6 cm 沿胫骨后缘下行至内踝尖弯向前，止于内踝前方，沿内踝骨折线向后剥离直达后踝骨折线，向前剥离显露踝穴内上角及胫骨下端前内侧面。再作腓骨后外侧切口，向前弧形延伸，止于外踝前下缘以显露下胫腓关节，若骨折位置较高，则单独显露下胫腓关节。在内侧切口向下用力牵引跟骨并向外推挤距骨...
