·临床研究·PDR患者硅油填充术后视网膜功能评价赵全良1,张春香2,勾明宝1作者单位:1(022150)中国内蒙古自治区牙克石市,内蒙古林业总医院1眼科;2内分泌科作者简介:赵全良,男,硕士,副主任医师,研究方向:糖尿病视网膜病变。通讯作者:张春香,硕士,副主任医师,研究方向:糖尿病并发症.zcx2008beijing@163.com收稿日期:2012-11-05摇摇修回日期:2013-04-01EvaluationofthefunctionsofretinaofdiabeticpatientsreceivingsiliconeoilinfluxingQuan-LiangZhao1,Chun-XiangZhang2,Ming-BaoGou11DepartmentofOphthalmology;2DepartmentofEndocrinology,InnerMongoliaForestryGeneralHospital,Yakeshi022150,InnerMongoliaAutonomousRegion,ChinaCorrespondenceto:Chun-XiangZhang.DepartmentofEndocrinology,InnerMongoliaForestryGeneralHospital,Yakeshi022150,InnerMongoliaAutonomousRegion,China.zcx2008beijing@163.comReceived:2012-11-05摇摇Accepted:2013-04-01Abstract誗AIM:Toknowthechangeoftheretinaandopticnerveforproliferativediabeticretinopathy(PDR)patientsaftersiliconeoilinfluxing,whohavebeenperformedwithsiliconeoilinfluxingfollowingvitrectomy,recoverthefunctionsoftheirretinaandopticnerve,andtogainaninsightintothesafetyofsuchoperation.誗METHODS:Totally30patients,whowereconfirmedasPDRandperformedwithvitrectomyoperationandsiliconeoilinfluxingduringJune2010toDecember2011,werechosenastreatgroup;and30cases,whowereperformedwithvitrectomyandC3F8influxingduetosufferingfromPDR,werechosenascontrolgroup.Thepatientsinthetwogroupswereobservedfortheirintraocularpressure(IOP),best-correctedvisualacuity(BCVA),aftersurgerycoloredfundusphotography,perimetry,etc.toexamineandevaluatetheretinalfunctions.誗RESULTS:Amongthe30casesinobservationgroup,15eyeswerefoundpaleopticnerve,9eyeswerefoundwithfaintcoloredopticnerve,and15eyeswerefoundwithlocalbloodvesselocclusionintheretina.Amongthe30casesinthecontrolgroup,thegaswasentirelyabsorbedafterthreemonthspostoperation,3eyeswerefoundwithfaintcoloredopticnerve,andnoonewasfoundwithpaintopticnerveandlocalbloodvesselocclusionintheretina.Visualacuity:Bycomparingthevisualacuityofthepatientspreoperationandpostoperationinthelatestfollow-upvisit,thevisualacuityofthepatientpostoperationinthesiliconeoilinfluxinggroupisimprovedby23eyesthanthatpreoperation,reducedby8eyesthanthatpreoperation,whilethevisualacuityof10eyeskeepsunchanged.postoperationintheC3F8group,thevisualacuityisimprovedby33eyes,reducedby3eyes(8.33%,P<0郾05),whichisofstatisticalsignificance.UsingGoldmanntenometertocomparetheaverageofthecasessevendayspostoperationandinthefollow-upvisit,thepressureinthesiliconeoilinfluxinggroupwas15.07依0.85mmHg,andthatintheC3F8groupwas17.84依1.86mmHg.Thechangepostoperationwasofsignificancedifference(P<0.05).Thepressureinoneweek蒺sfollowingvisitwasofnostatisticalsignificance.Visualfield:amongthe60caseswhowereperformedwithvitrectomyduetosufferingfromPDR,thevisualfieldexaminationof26casesthreemonthsaftertheoperationwasmade.Itwasfoundthat,theirvisualfieldwastubularchange.誗CONCLUSION:Diabeticpatientswithsiliconeoilfilling3monthsaftersurgerytoremovethesiliconeoilfoundinsomepatients,colorfadesorpaleopticlocalvascularocclusionoftheretina,thewhitesheath,proliferatinglikechange,Asaresult,thevisualacuityandvisualfieldofthepatientsshowedimpairmenttodifferentextent.Therefore,aftertheyareoperatedwithvitrectomyduetosufferingfromPDR,thesiliconeoilshouldbereleasedasearlyaspossible,dependingonretinalrestoresoastoavoidimpairmentofopticnerveandretina.誗KE...
