PermittoWork作业许可证1SCOPE范围1.1TheaimofthisprocedureistoprovideguidanceonthesitePermittoWorksystem.本程序的目的在于就现场作业的许可证制度提出指导性意见。1.2Thesitehandlesalargediversityofwork,sothepotentialforseriousaccidentsishigh.Topreventthemitisvitalthatthereshouldbesafesystemsofwork.TheywilloftenbeappliedandenforcedbymeansofaformalPermittoWorksystem.施工现场充满了高强度的作业，因此发生重大事故的可能性也比较高。为了防患于未然，必须制定一套安全作业系统。其中用得比较多的是一种正式制度是作业许可证制度。1.3AllCriticalOperationswillbeunderthecontrolofaPermittoWorksystemoperatedbytheSite.SpecifictrainingwillbegiventoallemployeesonthePTWsystem.Allpermitrequestsmustbemadeatleastcommencing.4hourspriortowork所有关键作业行为均必须处于现场作业许可证制度的控制之下。全体员工均必须接受有关作业许可证制度的具体培训。各种作业许可证都必须在作业开始之前至少提前四小时提出申请。1.4RoutineOperationswillnotbecoveredbyPermittoWork.常规作业不需要办理作业许可证。2TERMINOLOGY/DEFINITIONS术语/定义2.1PTW-PermittoWork作业许可证2.2CriticalOperations-Criticaloperationsincludehotworks,confinespacework,electricalwork,excavations,useofradioactiveandexplosivematerial,pressuretesting,liftingoperationsover50tonsareusing2cranesandanyotheroperationswhichmightbeidentifiedduringtheprogressoftheprojectthatneedtobecoveredunderthePermittoWorksystem.关键作业---关键作业包括高温作业、密闭空间作业、电气作业、挖掘、放射性和易爆炸材料的使用、压力测试作业、使用二台吊机起吊五十吨以上的设备以及项目进行过程中其它适用作业许可证制度的各类作业。2.3RoutineOperations–OperationsinareassolelydesignatedforoperationswillnotrequirePermittoWorkeventhoughcriticaloperationsarecarriedout.Forexampleaworkshoporgritblastingareawherehotworkisconsidered“normaloperation”willnotrequirePermittoWork.HowevertheseactivitiesmustbecoveredunderotherSSPsorspecialprocedureforthearea.Anyotherworkthatisnotcoveredunderthearea’sprocedurewillrequireaPermittoWork.常规作业----这是指那些在专属区域进行的、但不需要取得作业许可证就可以施工的作业（即使是关键作业）。例如在喷钢砂处理区域，高温作业是“常规操作”，所以就不必申请作业许可证。但是，在这些区域进行作业时，必须接受其它现场安全规定或适用WuxiprojectmanagementConsultantCo.,Ltd.无锡建设监理咨询有限公司Page1of4作业许可证该区域的特殊程序的控制。超出区域程序范围的任何作业，必须申请并获得作业许可证方可实施。3PROCEDURE程序3.1PermitDisplay许可证显示场所CopiesofaPermittoWorkshouldbeclearlydisplayed:应该在下列场合清楚地显示作业许可证的内容：Attheworksite.在作业场所。Inthepermitco-ordinationroom,withadditionalcopiesattheworksite.在许可证办公室，留有作业现场许可证的副本。3.2PermitSuspension许可暂时中止3.2.1IfforanyreasonworkcontrolledbyapermithastobesuspendedthenthepermitwillimmediatelybereturnedtotheSuperintendentwhowillacknowledgethefactandco-ordinatethereturnofallpermitstothePermitOffice.如果由于各种原因，原来受许可证控制的某项作业不得不暂时停止,则应该将许可证立即返还所在部门的主管，再由主管向许可证办公室通报真相，并将各种许可证退还给许可证办公室。3.2.2Exampleofreasonsforsuspensionare:许可证暂时收回执行的理由举例如下：Sitegeneralalarm.现场出现一般的报警。Operationalreasons.操作原因。Conflictofworkactivities.作业发生冲突。Expiryofperiodforwhichpermitisvalid.作业许可证有效期到期。Changeinworksiteconditions.作业现场的情况发生变化。3.2.3Aftersuspensionitwillbenecessary,oncepermissionhasbeengiventorecommencework,...
