激光治疗肝内胆管结石讨论论文 【摘要】目的探讨纤维胆道镜下 U100 激光治疗肝内胆管结石的方法及其安全性。方法肝内胆管结石术中应用胆道镜下 U100激光碎石治疗。结果 23 例中 20 例在胆道镜下 U100 激光碎石，共取出 28 枚结石，另 3 例因结石位于Ⅲ级胆管以上，胆道镜无法进入未能做 U100 激光碎石。术后 B 超复查，所有术中胆道镜下 U100 激光碎石术的病例肝外胆道未发现残余结石，未能做 U100 激光碎石术的 3 例肝内胆管结石仍在原处。术中胆道镜下 U100 激光碎石术后无胆漏及胆道出血并发症发生，无肝功能异常病例出现。结论胆道镜下 U100 激光碎石术安全可靠，效果确切。 【关键词】U100 激光碎石术胆道结石纤维胆道镜 【Abstract】ObjectiveToevaluatethetherapeuticeffectsandsafetyoftreatingintrahepaticbileductstonewithU100lasertherapy.Methods23patientswithintrahepaticbileductstonewastreatedwithU100lasertherapyandtheresultswereevaluated.Results20hadlaserlithotripsyundercholedochoscope,28stoneswereremoved,intheother3cases,U100lasertherapywasnotappliedsincethecholedochoscopecouldnotentertheintrahepaticbileductwherethestoneswere.Bultrasoundexaminationshowedtherewasnoremnantstonesinextrahepaticbileduct,thestonesnottreatedremainedattheoriginalplace.Therewerenobileleakage,hemobiliaorabnormalliverfunctionpostoperatively.ConclusionsTreatingintrahepaticbileductstonewithU100lasertherapyundercholedochoscopeissafeandeffective. 【Keywords】U100laserlithotripsybileductstonecholedochoscope 肝内胆管结石，一直是肝胆外科治疗的难点。2024 年 6 月至2024 年 4 月，本科利用纤维胆道镜和 U100 激光碎石技术，治疗肝内胆管结石 23 例，取得了较好疗效。现报告如下。 1 临床资料 1.1 一般资料 本组 23 例，其中男 10 例，女 13 例，年龄 32～６９岁(平均41.7 岁)；左肝内胆管结石 8 例，右肝内胆管结石 7 例，双侧肝内胆管结石 8 例。术前均经 B 超、CT 证实，Ⅱ级肝管结石 12 例，Ⅲ级肝管结石 3 例，Ⅱ、Ⅲ级肝管同时有结石 8 例。结石直径 0.6～1.2cm,无肝内胆管狭窄。其中 6 例因术前并发胆管炎，先予抗感染治疗，待炎症有所控制后再行手术。 1.2 治疗方法 激光机为德国 WOM 公司制造的双频双脉冲 U...
