AutoimmuneUrticaria:ImmunologicalMarkersJInvestigAllergolClinImmunol2011;Vol.21(7):546-550©2011EsmonPublicidadORIGINALARTICLEChronicAutoimmuneUrticaria:FrequencyandAssociationWithImmunologicalMarkersMAbdEl-Azim,SAbdEl-AzimMedicalMicrobiologyandImmunology,FacultyofMedicine,ZagazigUniversity,Zagazig,Egypt■AbstractBackground:Chronicautoimmuneurticaria(CAU),asubgroupofchronicidiopathicurticaria(CIU),ischaracterizedbysevereandpersistentwhealsaccompaniedbyrednessanditching.Diagnosisisalmostcompletelybasedonclinicalsuspicionandtheresultsoftheautologousserumskintest(ASST).Objectives:TodeterminethefrequencyofCAUandcomparetheclinicalandlaboratoryparametersofpatientswithpositiveandnegativeASSTresults.PatientsandMethods:Atotalof165patientswithchronicurticaria(CU)wereenrolled;31wereexcluded(knowncausesandpregnancy/breastfeeding),leaving134patientswithCIU.Aclinicalevaluationandroutineandspecificlaboratorytestswereperformed.Results:ThecauseofCUwasidentifiedin18.9%ofpatients;81.2%patientswereconsideredtohaveCIU.TheASSTresultwaspositivein39.6%ofpatientswithCIU,whohadmorefrequenturticariaattacksthanpatientswithanegativeASSTresult.Patientswithpositiveresultshadahigherurticariaactivityscorethanthosewithnegativeresults,althoughthedifferencewasnotstatisticallysignificant.Asforimmunologicalmarkers,theabsoluteeosinophilcountandserumimmunoglobulin(Ig)EtiterwerelowerinpatientswithapositiveASSTresultthaninthosewithanegativeASSTresult,although,again,thedifferencewasnotstatisticallysignificant(P=.07).AntithyroidantibodytiterandB-cellpercentagewerehigherinpatientswithapositiveASSTresultthaninthosewithanegativeresult,andthedifferencewasstatisticallysignificant(P=.04and.004,respectively).Conclusions:ASSTremainsabaselinediagnostictestforCAU.PatientswithCAUhadmorefrequentattacksandhigherantithyroidantibodytitersandperipheralB-cellpercentages,aswellaslowerabsoluteeosinophilcountsandserumIgEconcentrations.Keywords:Autologousserumskintest.Chronicurticaria.Autoimmunity.■ResumenAntecedentes:Laurticariacrónicaautoinmunitaria(UCA),unsubgrupodeurticariacrónicaidiopática(UCI),secaracterizaporlapresenciadehabonesdecaráctergraveypersistenteacompañadosdeenrojecimientoyprurito.Eldiagnósticosebasacasiporcompletoenlasospechaclínicaylosresultadosdelapruebacutáneaconsueroautólogo(PCSA).Objetivos:DeterminarlafrecuenciadelaUCAycompararlosparámetrosclínicosydelaboratoriodepacientesconresultadospositivosynegativosenlaPCSA.Pacientesymétodos:Seincluyóa165pacientesconurticariacrónica(UC),delosqueseexcluyóa31(causasconocidasyembarazo/lactancia),quedando134pacientesconUCI.Sellevaronacabounaevaluaciónclínicayanálisisderutinayespecíficos.Resultados:LacausadelaUCseidentificóenun18,9%delospacientes;seconsideróqueun81,2%depacientespadecíaUCI.ElresultadodelaPCSAfuepositivoenun39,6%depacientesconUCI,queteníanepisodiosdeurticariaconmayorfrecuenciaquelospacientesconunresultadonegativoenlaPCSA.Lospacientesconresultadospositivospresentaronunapuntuacióndeactividaddeurticariamáselevadaqueaquellosconresultadosnegativos,sibienladiferencianofueestadísticamentesignificativa.Encuantoalosmarcadoresinmunológicos,elrecuentoabsolutodeeosinófilosyeltítulodeinmunoglobulina(Ig)EensuerofueronmásbajosenpacientesconunresultadopositivoenlaPCSAqueenaquellosconunresultadonegativoendichaprueba,aunque,denuevo,ladiferencianofuees...
