房屋鉴定报告房 屋 安 全 鉴 定 报 告Building Safety Appraisal Report项 目 负 责 人Project Manager审核人Audited by批准人Approved by签 发 机 构Issuing Agency发 文 日 期Issuing Date年月日剖断申报包含以下内容：1. 根本情况。2. 房屋概况。3. 现场检测成果。4. 工程检测成果。5. 复核计算成果。6. 房屋破坏原因分析。7. 剖断结论及处理看法。8. 本剖断申报共 5 页。This appraisal report include the following contents:1、 General information.2、Overall conditions of the building.3、Results of site-testing.4、Results of engineering testing.5、Results of recomputation.6、Analysis of the causes of building damages.7、Conclusion of the appraisal and treatment recommendations.8、This appraisal report consists of5pages in total.注：申报内容中4、5 项不是每处房屋剖断都须要，应视实际产生而定。Note: Item 4 or 5 listed above is not required in all building appraisals and should be provided ona case-by-case basis.注意事项1、剖断结论根据《危险房屋剖断标准》(JGJ125—99）、《建筑抗震剖断标准》（GB50023—95）和有关技巧规范提出。2、项目负责人、审核人、赞成人三级签字均应相符《北京市房屋安然剖断工作治理办法》的请求，签发机构应填写剖断机构的全称，并加盖公章方为有效。3、剖断结论为安然应用的，房屋安然剖断申报自签发之日起一年内有效，当剖断为危险房屋或局部危险房屋时，请按解危时限及时处理。4、一般情况本申报仅对考验时的房屋近况负责。5、本申报结论有贰言时，请于剖断申报签发之日起十五日内持本剖断申报向北京市房屋剖断治理所书面申请复核剖断。Notices1、The conclusion of this appraisal report is provided on the basis ofthe Standardof Dangerous Building Appraisal (JGJ125-99) 、Standard for Seismic Appraiser ofBuildings (GB50023-95), as well as other relevant technical codes.2、Thisreport must be duly signed by the three levels of authority.
