手部高压喷漆枪注射伤的治疗【摘要】 目的 探讨手部高压注射伤的治疗方法和效果。方法 我院共收治高压注射伤病人 15 例，均系男性，年龄 18～51岁，平均 25 岁。13 例一期清创闭合伤口。2 例扩创，彻底清创，伤口敞开 3～5 天后做二期处理闭合伤口。结果 13 例一期闭合伤口的患者中 2 例出现伤口部分皮肤坏死，清除坏死皮肤经十余天换药后伤口闭合。结论 2 例伤指感觉减退。8 例功能基本恢复。2 例二期闭合伤口，术后恢复良好。【关键词】 高压注射伤；清创；喷漆枪 【Abstract】 Objective To study the treatment of the procedures and outcome of high-pressure injection 15 cases are all males, the mean age was 25years (range 18to51 years).13 of them were treated with debridement and direct closure; 2 of them were treated with debridement and closure 3-5day 13 cases treated with debridement and direct closure, 2 of them was skin necrosis in part wound. Cut off the inactive skin, after 20 days changing of dressing, wound closed 2 of them esthesia decreased of the wounded finger. Other cases recovered all functions. 【Key words】 high-pressure injection injuries；debridement；spray gun 随着工业,特别是机械制造业的迅速进展,高压喷漆枪作为一种较先进的生产工具得到了较为广泛的应用,而由此所致的高压喷漆枪伤成为近年来一种新的外伤类型-手部高压注射伤（high-pressure injection injuries ,HPII)。该病是手外科临床少见的特别急症。由于一般患者早期症状轻，易被忽视，延误及时、正确的治疗。因其致伤物兼具机械损伤、高压冲击和化学损害,故高压喷漆枪伤的临床表现、治疗及预后均有其自身特点［1～3］。我院自 2000年以来共收治高压喷漆枪伤 15 例,现将其临床特点加以总结和分析。 1 临床资料 一般资料 本组 15 例均系男性,年龄最小18 岁,最大 51 岁,平均 25 岁。累及部位:左上肢 12 例,其中前臂 3 例,手掌 5 例,拇指 1例，示指 1 例,中指 2 例;右上肢 3 例,均为手掌。13 例为伤后 6h 内入院，2 例单指受伤 24h 后入院。15 例均为高压油漆喷枪致伤。 治疗方法 损伤在 12h 内的患者均于臂丛麻醉下接受急诊清创术，术前患肢...
