标准外伤大骨瓣减压术治疗重型颅脑损伤的临床效果【摘要】［目的］探讨标准外伤大骨瓣减压术治疗重型颅脑损伤的临床效果。［方法］对 90 例重型颅脑损伤患者均采纳标准外伤大骨瓣开颅减压术治疗并观察其治疗效果。［结果］伤后 1 年随访，生存75 例，其中 50 例(％)预后良好，中残 15例(%)，重残 5 例(％)，植物生存 2 例(％)，死亡 18 例(％)。其中死亡病例基本为瞳孔散大或呼吸不规则或 CT 示：环池、鞍上池消逝者，病因基本为多器官功能衰竭、肺部感染等。［结论］标准外伤大骨瓣减压术可使脑组织得到充分减压，有效保护脑功能，明显改善重型颅脑损伤患者的预后，降低病死率。【关键词】颅脑损伤外伤大骨瓣减压脑挫裂伤硬膜下血脑内血肿 Abstract：［Objective］Toexploretheclinicaleffectofstandardtraumabigboneflappressurereductionoperationonseverebraininjury.［Method］90casestakethesaidtherapy，thenobservethecureeffect.［Result］Makefollow upafteroneyear，75casessurvived，amongwhich，50(％)hadgoodprognosis，15(%)middlehandicapped，5(％)severelyhandicapped，2(％)livedasplant，18(％)thedeadones，theybasicallyhadplatycoriaorabnormalbreath，ortheirCTshowedmissingsicternaambiensanduppersellaambiens，mostlycausedbymulti organfailureandlunginfection，etc.［Conclusion］Thesaidtherapycanfullyreducethepressure，effectivelyprotectbrainfunction，markedlyimprovetheprognosisofthesaiddisease，anddecreasedeathrate.
