524ChinOphthalmicRes，June2010，Vo1．28，No．6先天性视网膜劈裂症自然病程的临床观察·临床研究·辛梅张关霞张军军圄ClinicalobservationofnaturalcourseofcongenitalretinoschisisX／NMei，ZHANGMei—xia，ZHANGJun-jun，{DepartmentofOphthalmology，WestChinaHospital，SichuanUniversity，Chengdu610041，ChinaAbstractBackgroundCongenitalretinoschsisisrareX-linkedinheriteddisorder，mainlyaffectsbilateralretinaandisseldominclinic．Someseverecomplications，suchasretinaldetachmentandvitreousbleeding，remaindificuhtotreat．0bjectiveTheaimofthisstudywastoobservethenaturalcourseofclinicaldevelopmentinpatientswithcongenitalretinoschsis．MethodsThiswasaretrospectivecase—observationstudy．Theclinicaldataandfunduscharacteristicsof32eyesfrom16caseswithcongenitalretinoschsiswereanalyzed．ThesepatientswerediagnosedinWestChinaHospitalthroughfluorescencefundusfluoresceinangiography(FFA)andopticalcoherencetomography(OCT)．ResultsAllofthe16casesofcongenitalretinoschesiswerebilateralwiththefoveoschisis．Peripheralretinoschisiswasfoundin12eyesof6cases．Thepatientsagedfrom6to29yearsoldwiththeaverageage14．2years．Duringthefollow—upperiod，3eyesdevelopedmacularhole，and3eyesprogressedretinaldetachment，and1eyedevelopedvitreoushemorrhage．Themaculawasinvolvedin1eyewithperipheralretinoschisis．Twenty-foureyespresentedthestablecourse．ConclusionThestateofillnessinmajorityofpatientswithcongenitalretinoschisiscanbestabilizedforalongduration．Butsomecomplicationsandillnessprogressarefoundinpartialpatients．Regularfollow—upshouldbeperformedforthepatientswithcongenitalretinoschsis．OCTisausefultoolforthemonitoringthemanifestationoffundusincongenitalretinoschsis．Keywordscongenitalretinoschisis；naturalcourse；opticalcoherencetomography摘要目的观察先天性视网膜劈裂症的自然病程。方法回顾性病例研究。对确诊为先天性视网膜劈裂症患者16例32眼的临床资料、眼底检查、荧光素眼底血管造影(FFA)和光学相干断层扫描(OCT)等进行分析，随访0．5～6a。结果l6例患者均为双眼发病，均有黄斑中心凹劈裂，6例患者合并周边部视网膜劈裂，年龄6～29岁，平均14．2岁。随访中，3眼中心凹劈裂进展为黄斑裂孔，3眼进展为视网膜脱离，1眼玻璃体积血，1眼周边视网膜劈裂向赤道部进展，24眼未见明显改变。结论先天性视网膜劈裂症虽可长期保持稳定，但部分患者病情可进展，应定期进行随访。OCT可作为随访检查的主要工具。关键词先天性视网膜劈裂症；自然病程；光学相干断层扫描中图分类号R774．1文献标志码A先天性视网膜劈裂症为双眼发病的性连锁隐性遗传疾病，是发生在视网膜神经纤维层内神经上皮的层问分裂。以男性多见，常在儿童期发病，可影响视力。本病发病率低，为1：5000～1：25000，临床较为少见，一般发展缓慢，但可发生视网膜脱离(retinaldetachment，RD)及玻璃体出血等并发症。本研究主要阐述先天性视网膜劈裂症的临床特征及自然病程，报道如下。DOI：10．3969／j．issn．1003-0808．2010．06．012基金项目：教育部重点项目(108178)作者单位：610041成都，四JII大学华西医院眼科通讯作者：张美霞(Email：zhangmeixia@medmail．conlal1)1资料与方法对2002-2008年在四川I大学华西医院眼科就诊，经眼底检查、荧光素眼底血管造影(fluorescencefundusangiography，FFA)及光学相干断层扫描(opticalcoherencetomography，OCT)检查的所有病例进行回顾性分析，确诊为先天性视网膜劈裂症的患者l6例32眼，对其临床及随访资料进行分析。16例均为男性患者，双眼发病；年龄6～29岁，平均14．2岁；均以视力下降为主诉就诊；无明确家族史。所有患者均进行视力、裂隙灯显微镜、间接检眼镜、眼底照相、FFA及OCT检查，随访0．5～6a。视网膜劈裂症患者累及黄斑区，50％的患者发生周边部视网膜劈裂...
