Date日期:2013年6月21日June21,2013PRIVATE&CONFIDENTIAL机密INTERNSHIPPLACEMENTAGREEMENT管理实习生实习协议ThisAgreementismadebetweenXXXXX(The“Trainee”),PassportNo.XXXXXXXXandXXXXXXXXXXXXXXXXXXXXXXXXXXXXX(The“Hotel”),basedonthefollowingtermsandconditions:本协议的协议双方为XXXXX(以下简称实习生),护照号码XXXXXXXX和XXXXXXXXXXXXXXXXXXXX基于以下条款而签订;1InternshipPeriod:TheinternshipperiodwillbefromXXXXXXXtoXXXXXXXXXXX.TheTraineewillbeattachedtotheHotelasmanagementtraineeandtoassistinthelanguagetrainingforHotelstaff,ifdeemednecessary.实习期:实习的开始日期自XxXXXXX至XXXXXXXXX止,以管理实习生的身份在酒店相关部门实习,并根据运作需求协助酒店员工的语言培训。2TrainingAllowance:AnamountofUSD800.00grossoftax(ConvertedtoRMB5000)permonthwillbepaidinarrearsduringtheinternshipperioddirectlytotheTrainee.培训津贴:税前每月美金800元(兑换相应人民币5000),将在每月月末直接付给实习生。3Tax:TaxonthetrainingallowanceasassessedbytheGovernmentofthePeople’sRepublicofChinawillbetheresponsibilityoftheHotel.AnyothertaxliabilitiesasassessedbyanygovernmentwithinoroutsideofthePeople’sRepublicofChinawillbeTrainee’sownresponsibility.税:酒店方承担实习生根据中华人民共和国的标准的个人所得税。其他中华人民共和国政府或其他政府要求的税金由实习生个人承担。4PublicHoliday:TheTraineewillbeentitledtopublicholidaysasdecreedbytheGovernmentofthePeople’sRepublicofChina.公共假期:实习生将享有中华人民共和国规定的法定假期。5DayOff/WorkingHours:TheTraineewillbeentitledtotwodaysoffperweek.Theaverageworkinghoursis40hoursperweek.休息/工作时间:实习生每周可休息2天,每周平均工作时间为40小时。6Vacation:TheTraineewillbeentitledtooneworkingdayperfullmonthworkedpaid1holidayaftercompletionoftheinternshipperiod.休假:在完成实习期之后,实习生可享有每月一天的有薪年假。7Accommodation:TheTraineewillbeprovidedwithtwinshareaccommodationinhotelroomduringthetermofthetraining住宿:实习期间,提供酒店双人房住宿。(带卫生间)。8Meals:Thehotelshallprovideallthebreakfastsandthreedinnerseveryweek(includingonenon-alcoholdrinkeachtime)inCaféUno.Lunchesandotherdinnerswillbeprovidedinstaffcafeteria.酒店向培训生提供三餐,早餐和每周三次晚餐,包括无酒精饮料,在西餐厅使用,午餐及其它晚餐在员工餐厅使用。9Laundry:ThehotelwillprovidefreeuniformlaundryandmaximumRMB500permonthforPersonallaundry(Sale’sprice)attheHotellaundry.酒店提供免费的工作服洗涤及每月限额为500元的个人洗衣和干洗服务。10Insurance:TheHotelwillbuyaccidentinjuryinsurancefortheTraineeduringtheinternshipperiodandwillprovidefreemedicaltreatmenttothetraineeinitsClinicforcommonillnessesonly.保险:在实习期间,酒店为实习生购买工伤保险,对于普通疾病将在酒店医务室享受免费治疗。AirPassage:TheTraineewillberesponsibleforthecostsofalltravelexpensesfromhisorherhomecountrytoHotelpriortocommencementofinternshipperiodandfromChangshatohisorherhomecountryafterthecompletionoftheinternshipperiod.航空机票:在实习开始和结束时,实习生将自行承担从所在国家到酒店的往返差旅费。TrainingReport:TheTraineewillberequiredtosubmitmonthlyreportstoHotelManagementbeforethe5thofeverymonthduringtheinternshipperiod.培训报告:实习生需要在每月1-5号向酒店管理层提交每月实习报告。VisaProcedures:签证程序:Step1:PrepareForeignerEmployeePermitbyyourself.第一步实习生自己办理外国人就业证。Step2:ApplyforVocationVisaEntryatChineseEmbassywithbothEmployeePermitandVisaNotificationsentbyhotel.第二步实习生携带外国人就业证和签证通知前往中国...
