EXPERIMENTALANDTHERAPEUTICMEDICINE5:507-510,2013Abstract.Theaimofthisstudywastoinvestigatetheeffectsofphacoemulsificationwithintraocularlens(IOL)implanta-tiononintraocularpressure(IOP)andanteriorchamberdepth(ACD)inpatientswithcataractorcataractassociatedwithprimaryangleclosure(PAC).Atotalof361patients(481affectedeyes)withsenilecataract(cataractgroup)and44patients(52affectedeyes)withcataractassociatedwithPAC(cataractwithPACgroup)underwentphacoemulsifica-tionwithIOLimplantationfromJuly2005toMay2007andwerefollowedupfor3to25months.Therewasasignificantdifferencebetweenpre-operativeandpost-operativeIOPs(t=9.270,P<0.01)inthecataractgroupandinthecataractwithPACgroup(t=3.29,P<0.01).Nosignificantdifferenceswereidentifiedinpre-operativeIOP(t=-2.437,P>0.05)andtheIOPthreemonthsaftersurgery(t=2.154,P>0.05)betweenthetwogroups.Therewasasignificantdifferencebetweenthepre-operativeandpost-operativeACDs(t=7.781,P<0.01)inthecataractgroupandinthecataractwithPACgroup(t=4.528,P<0.01).AsignificantdifferenceinACDbetweenthetwogroups(t=8.325,P<0.01)existedpriortosurgerybutfollowingsurgery,theACDsofthetwogroupswerenotsignificantlydifferent(t=2.86,P>0.05).PhacoemulsificationwithIOLimplantationhasIOP-loweringeffectsoncataractandcataractwithPACpatients.TheInternationalSocietyofGeographyandEpidemiologyofOphthalmologyclassificationmethodforangleclosureglaucomawasadoptedinourstudy.FurhterstudiesarerequiredtoprovethesafetyandmechanismofloweringIOPimpactofphacoemulsifationtowardsPACglau-coma(PACG).IntroductionTheincidenceofcataractandglaucoma,includingprimaryangleclosure(PAC),graduallyincreaseswithage.Somepatientssufferfrombothcataractandglaucoma.Inthetreat-mentofPAC,phacoemulsificationandintraocularlens(IOL)implantationhavebeenused.Theoccurrenceanddevelopmentofdiseaseriskfactors,theanteriorchamberangleofPACandthedegreeofintraocularpressure(IOP)arereportedtobesignificantlyimprovedbyphacoemulsification(1-8).Accordingtotheclinicalsituation,cataractandglaucomamaybetreatedbyasimplecataractsurgery,surgicalstaging(wherecataractsurgeryiscarriedoutbeforeglaucomasurgeryorviceversa)orcombinedsurgery(9).ThegeneralriskfactorsforPACglaucoma(PACG)include:familyhistoryofangleclosure(10,11),age(12,13),gender(12-14)andAsianorInuitdescent(15-19).Otherriskfactorsincludeeyehyperopia(20-22),theperimeteroftheshallowanteriorchamber(20,22,23),theperimeterofthecentralshallowanteriorchamber(22,24),thesteepnessofthecornealcurvature(25),thethicknessofthelens(22,26)andtheaxiallength(22,26,27).Thelenselementhasbeengainingwidespreadattention.Astudyhasshownthatthelensweighs~205mgin20-year-oldindividualsandupto285mgin80-year-olds,agrowthof>30%.Thethicknessofthelensattheageof20is~4mmandattheageof65,itmayreach4.8mm,a20%increase(28).Therefore,inPACwithcataract,cataractremovalbysurgeryisexpectedtoprovidenewoptionsonanteriorsegmentstructureanditsfunctionimprovement.Weanalyzed481eyesamong361patientswithcataractand52eyesamong44cataractpatientswithPACtoobservetheeffectofphacoemulsificationcataractsurgeryonIOPandcentralanteriorchamberdepth(ACD).PatientsandmethodsGeneralinformation.FromJuly2005toMay2007,481eyesin361patients(cataractgroup)undergoingsenilecataractphacoemulsificationandIOLimplantationwereinvestigated.Atotalof144males(193affectedeyes)and217females(288affectedeyes)wereincluded.Therewere44patients(52eyes)whohadcataractwithPAC(cataractgr...
